Gap-fill semantics

fromSeq is inclusive. To fetch the first missing event after contiguous id N, send fromSeq=N+1 (last_contiguous_event_id + 1). Alternatively, send cursor=N; cursor is exclusive. Never send both parameters.

Process events in ascending eventId order and dedupe by eventId together with the returned execution identity. HTTP 409 means the requested replay point is outside retention or integrity cannot be proven; take a fresh account bootstrap, reconcile open orders, apply scoped cancel-all where needed, and reseed quotes.
